FEES
Please contact our office for additional information regarding fees for services provided. We accept the following payment methods: private payment, Employee Assistance Program (EAP), and most major insurance provider reimbursement.
*Unfortunately, we do not have sliding scale fees and do not accept Illinois Medicaid.
